I will be attending the June 27 test session in Peoria

Forms should always be mailed to the hosting club. It does not matter which club you are a member of. Forms should be in the hands of the test chair 2 weeks prior to the test date and to be sure your skater gets a spot you should have them in sooner than that. You must be a current member of USFS to test.
